SI Bhimaa is transferred to Mahendragiri in Karnataka—a town ruled by ruthless gangsters. Determined to clean up the lawless region, Bhimaa soon befriends Vidya, a school teacher and devotee of the respected Ayurvedic practitioner, Ravindra Varma. However, Bhimaa discovers the shocking truth about Ravindra Varma one night when he intercepts a tanker smuggling young children.

He learns that Ravindra Varma is conducting horrifying illegal experiments to create "Sanjeevini," a mythical herbal formula capable of reviving the dead. When Bhimaa confronts him at his hideout, Ravindra Varma captures Vidya and the children as hostages. Taking advantage of the situation, Ravindra Varma brutally murders Bhimaa, Vidya, and the innocent children.

Meanwhile, Bhimaa’s twin brother Ramaa, a gentle temple priest, is devastated to learn of Bhimaa’s death. Overcome with guilt for failing to understand his brother, Ramaa becomes the vessel for Bhimaa’s vengeful spirit. Empowered by Bhimaa's soul, Ramaa tracks down Ravindra Varma and kills him, avenging Vidya and the children and restoring justice to Mahendragiri.
