Join us in this lecture where Professor Zach Murphy will be discussing the vestibular pathway of the vestibulocochlear nerve, cranial nerve VIII. We begin with the peripheral apparatus, describing the specialized structures within the vestibule and semicircular canals that detect linear and angular acceleration. This includes the maculae of the utricle and saccule, which sense head position and linear movements, and the cristae ampullaris of the semicircular canals, which respond to rotational movements. These mechanical stimuli are converted into electrical signals by hair cells, which synapse with bipolar neurons of the vestibular ganglion.
We then follow the central pathway as the vestibular nerve fibers project to the vestibular nuclei in the rostral medulla and caudal pons. From here, we explore the multiple projection routes—ascending to the extraocular motor nuclei via the medial longitudinal fasciculus to coordinate eye movements (vestibulo-ocular reflex), descending via the vestibulospinal tracts to maintain posture and balance, and projecting to the cerebellum for fine-tuning motor coordination. We also discuss cortical projections for spatial orientation and perception of movement.
The lecture highlights how the vestibular system integrates with visual and proprioceptive inputs to maintain equilibrium and gaze stability. We conclude by briefly reviewing how disruptions to this pathway can manifest as vertigo, nystagmus, balance disturbances, and motion sickness, while emphasizing the importance of vestibular testing in clinical practice.
