We recommend that the student's parent or guardian is with the student when s/he clicks the Google Form "submit" and "view results" button so that the the parent or guardian can see -- in real time -- how the child is progressing. We are optimistic that if the parent or guardian sees where the student's problems are, then the student's performance will improve.
