In May 2021, the Colonial Pipeline, which supplies nearly half of the fuel to the U.S. East Coast, was hit by a ransomware attack that forced the company to shut down its operations for several days. The breach began with a compromised password tied to an old VPN account that had been left active even though it was no longer in use. The account also lacked two-factor authentication (2FA) — meaning the attackers only needed a single stolen password to gain entry.
Once inside, the attackers installed ransomware that locked Colonial Pipeline’s data and systems, demanding payment in exchange for restoration. The company ultimately paid about $4.4 million to regain access. During the shutdown, millions of people felt the impact: gas stations ran out of fuel, transportation slowed, and prices increased across multiple states. It was one of the most disruptive cyber incidents in U.S. history, and all of it began from a small, overlooked weakness.
The Colonial Pipeline attack shows how a simple human oversight can trigger massive real-world consequences. There was no high-level hacking involved — just a forgotten account and a missing security step. Many organizations have similar risks: inactive user accounts, old software, or systems that haven’t been checked in years.
What makes this story important is that it’s completely preventable. A few small actions — disabling unused accounts, requiring 2FA, and monitoring for unusual logins — would have likely stopped this attack before it began. It’s a strong reminder that cybersecurity starts with small details, and every person who manages an account, password, or device plays a part in keeping their workplace secure.
